The Emergency Form is required for all campers who do not already attend Our Lady of Grace Catholic School.
Medication authorization is required only for campers who will need medication administered to them during camp hours.
FARE Emergency Plan is required only for campers who have a Food Allergy & will need medication administered to them during camp hours, in the case of an Emergency.
Asthma Action Plan is required only for campers who have Asthma and or if they will need medication administered to them during camp hours.
Authorized Pick-up is required for all campers.
All Campers Photo Release Form should be returned. If you would prefer your camper not to have photo or video released. Please Sign their name at the bottom of the page with No Photo/video.
